Outreach Coordinators are responsible for all functions associated with community service programs, coordination of program services, customer orientation, referrals, eligibility determination, and program reporting.

>Protect the privacy of customers/families and hold in confidence all information obtained in the course of service.
>Complete intake applications for a variety of programs to determine eligibility and complete necessary paperwork. Make appropriate referrals as deemed necessary. Perform necessary follow up to assure services are received.
>Locate and assess the needs of customers and refer them to appropriate resources or services.
>Maintain current and accurate household service records and ensure they are integrated into the Agency’s master file system and computerized data tracking system.
>Work with Outreach Director to design, implement and coordinate special projects (i.e. Walk for Warmth) designed to fill unmet needs and service gaps in severely impacted areas including conducting Outreach Committee/Walk for Warmth steering committee meetings.
>Provide reports on activities and data from special projects and activities by gathering information, compiling reports and distributing findings.
>Establish and maintain effective working relationships with local resource agencies, service providers, employees and the general public to prevent duplication of services.
>Participate in personal and professional development activities.
>Recruit and coordinate the involvement of volunteers in program activities.
>Participate in personal and professional development activities
>Perform other duties as assigned.
Education and Experience Qualifications:
>Must have a Bachelor’s Degree in a human service field or equivalent or experience in a related field.
>A minimum of three years work in human services, case management skills and previous supervisory experience.
>Must possess the ability to empathize and communicate with economic and socially disadvantaged groups; ability to work well with others on all levels of the human spectrum; sensitive to the needs of others.
>Should possess interviewing skills and the ability to build trust and rapport with customers.
>Must have the ability to understand and follow oral and written instructions.
>Must have proficient computer skills (Word, Excel, Outlook, Internet)
>Demonstrate communication skills both oral and written.
>Must have reliable transportation, valid driver’s license, provide proof of insurance, pass State Police criminal clearance check and MMCAA’s “Insurance Carriers” driving record review.
>Must be self-directed and able to coordinate the completion of assigned tasks and be willing to accept new challenges on a daily basis.
>Comply with Agency’s smoke free and drug free policies.
>Understand the importance of daily work attendance.
